Allow us to specify a note on block list/allow list entries to associate with either ticket numbers if we wish or provide any context as to why something is allowed.

While we often have tickets for the items that need to be put in place, we have no way to reference it so we can see the justification of why it's there or if it's still needed when we audit and see if there is anything that could be cleaned up.
